Suicide inquest hears detectives who concluded Perth woman Amy Wensley killed herself admit errors

A detective has admitted in the Perth Coroner’s Court that he failed to properly investigate the death of a woman who he believed had taken her own life.

A coronial inquest is being held into the shooting death of Amy Wensley in Serpentine in 2014.

Detective Sergeant Tony Kirkman attended the scene, deemed Ms Wensley’s death was a suicide and the house was cleaned the following day.

The court has heard from uniformed officers who also attended the scene on the day, and believed the death was suspicious.
