Hillier murder inquest told SA child protection department had opportunities to save children

The deaths of two children murdered by their mother’s partner in Adelaide’s north five years ago could have been avoided if the Department for Child Protection had not “failed” in its obligations to investigate, a coronial inquest has heard.
