Current medical guidelines rely on a highly variable evidence base, from highquality clinical trials to expert opinion. Currently, only 18% of Australian clinical guidelines addressing the top10 causes of mortality are based on level I (or equivalent) evidence.1 This shortfall must be addressed to optimise clinical outcomes for patients, with the benefit of economic and healthcare system efficiencies.
