Damning findings over slaying of pregnant woman by ex-partner on the Gold Coast

Authorities failed to identify or act on “numerous risk factors” before Brock Wall brutally murdered his ex-girlfriend and their unborn child with an axe, a coroner has found.
Wall also told DVConnect Mensline he was going to kill Fabiana Palhares but the call taker did not record this information or notify anyone of the threat, Deputy State Coroner Jane Bentley revealed in findings released on Wednesday.
