The mistake was made on Wednesday by vaccination staff at a Melbourne aged care facility, but was only revealed on Friday afternoon.
A refrigeration error has forced health authorities to dump 125 doses of the Pfizer vaccine.
The Federal Health Department confirmed the blunder on Friday afternoon.
The decision to dump 25 vials of the vaccine, which contained enough doses for 125 people, came after health authorities couldnt confirm whether they had been kept at the correct temperature.
Although the bungle occurred on Wednesday February 24, it is only now being reported by the government.
