information provided by medical professions in relation to the risk that the mother’s drug use posed to T’s safety,” she wrote in her conclusion.

“Doctors and nurses warned the department that the mother was using
drugs and had fallen asleep on top of T but the department did not adequately assess that information.

“Had that information been given the significant weight it deserved, it is highly likely that T would have been removed from his mother’s care prior to his death.”
